Document required at the time of Admission:

 

1)    Attested Copy of the Birth Certificate of child, issued by the municipal corporation.

2)    Copy of ID proof of parents/guardian.

3)    Medical Fitness Certificate of child by registered medical practitioner.

4)    Copy of proof of Residence.

5)    Fee receipt  of Sibling (if applied  under    sibling criteria).

6)    Four Additional passport size photos of child.

7)    Copy of Aadhar card of child

8)    Caste certificate (SC/ST/OBC) (if applicable)

9)    Proof of guardianship (if applicable)

10) Produce any other documents validating data entered in the form.
